What is Hospital at Home?
The goal of a hospital at home program, sometimes called a virtual hospital, is to avoid hospitalization and manage higher-acuity, higher-risk patients within their own home with hospital-standard care. Patients may be identified for a hospital at home program upon presentation to the ER, by their primary care physician, or while an inpatient. Evidence shows that hospital at home programs produce better clinical outcomes, improve patient satisfaction, and reduce the cost of care.
Does CMS reimburse for Hospital at Home?
As of November 2020, CMS will reimburse for acute hospital care provided at home as part of a new, innovative Acute Care at Home model. This allows regulatory flexibility for hospitals to deliver inpatient care at home, while still receiving hospital payments This new program is an expansion of the prior Hospital without Walls model.
